You must fill out an Invention Disclosure Report (IDR) form when making a submission of any kind (patentable invention, copyrightable software, biological material, etc.) to WARF.
WARF cannot process your invention or consider it for patenting and licensing until we receive a completed IDR from you. For information on how to fill out an IDR form, see Disclosing to WARF.
Invention Disclosure Report Form (Word document format)
After opening the IDR in MS Word, type your information directly into the form. Then send the IDR to your intellectual property manager through our secure email page.

Use this format if you wish to download the IDR. After downloading the IDR and opening it in MS Word or another word processing program, type your information directly into the form. Then send the IDR to your intellectual property manager through our secure email page.
